The investigations of the accidents contained in this listing are conducted by MMS district and regional personnel. Some may also involve headquarters, U.S. Coast Guard, and other Federal agency personnel. These accidents are major in nature and normally involve fatalities, serious injuries, substantial property damage, or a significant pollution event. The investigative report contains information on the circumstances surrounding the accident, the root cause or causes of the accident, and recommendations to prevent a recurrence of such an incident.
